Housing Program Case Manager – The Relatives

The Housing Program Case Manager provides a variety of office and field activities to manage and monitor a rapid re-housing/transition-in-place program for young adults. Performs direct client services ,complies with related documentation and implements the RENEW Model with housing clients.

Work involves providing supportive services to promote participants self-sufficiency, integration into the community, and permanency in housing, performing administrative tasks involved in the review and maintenance of a caseload of program participants. The principal duties are performed both in a general office environment and in the field and community where program participants reside. Apply now.

On Ramp Transition Specialist

The person in this role will coordinate the services for young adults in transition. This may include working with various agencies, providers, faith based organizations, and individual partnerships. They will develop, implement, and manage the individualized plans for young adults ages 16-24. They will help to ensure young adults are provided with the tools, support, and resources necessary to become self sufficient. Apply Now.

On Ramp Resource Center Assistant- (Full-Time)

The person in this role will complete all necessary paperwork and documentation, coordinate monthly workshops and program activities, and complete futures plans, client assessments, team meetings. Full-Time Crisis Counselor 

Interested in getting hands-on experience in the human services field? The Relatives is seeking qualified applicants to serve in our Youth Crisis Center. Staff person would be responsible for interacting and leading activities with clients (ages 7-17), inputting accurate weekly data of all crisis calls and walk-ins into the CDT system; supervising clients throughout their shift; completing intakes if they arrive during their shift; ensuring the house is clean and tour ready; administering medications as needed; and other duties as needed. High School Diploma/ Equivalent Required. Experience with population preferred. The Youth Crisis Center operates 24/7, 365. Must be able to attend a two week orientation. Must be +21. Apply now.

Part-Time Crisis Counselor
