The A Way Home Housing Case Manager provides a variety of office and field activities to manage and monitor a rapid re-housing program for pregnant and parenting young adults. Employee will provide direct client services, complete related documentation and implement the TIP Model of case management with housing clients.
Work involves providing supportive services to promote participants self-sufficiency, integration into the community, and permanency in housing, performing administrative tasks involved in the review and maintenance of a caseload of program participants. The principal duties are performed both in a general office environment and in the field and community where program participants reside.

The Program Manager oversees both the Housing and On-Ramp programs, providing strategic direction and operational oversight, while supervisors handle daily operations. Key duties include ensuring objectives are met, maintaining high-quality services, implementing the TIP Mode of Care, and fostering a collaborative work environment. The manager also builds community partnerships to increase program impact and is part of The Relatives Senior Leadership Team, advancing the organization’s mission.

The On-Ramp Resource Center Supervisor plays a pivotal role in the efficient functioning of The Relatives On Ramp Resource Center, a welcoming drop-in facility that caters to the diverse needs of young adults between the ages of 16 and 24. Operating on weekdays from 9 am to 5 pm, this center serves as a crucial hub addressing a range of concerns, including housing, education, employment, health, safety, and community integration. This position is integral for extracting and analyzing data about the organization’s programming results and ensuring that the program meets all targets and goals. Lastly, this position will ensure compliance with all applicable regulations.
Program Case Manager Supervisor

The Program Case Management Supervisor plays a key role in supporting young adults through both strategic leadership and direct involvement. You will set case management direction aligned with organizational goals, mentor case managers, provide TIP Model training, and ensure compliance with documentation standards. Alongside these duties, you’ll manage a caseload of 7-10 clients personally and promote ongoing team development through coaching and feedback. The position also focuses on measuring and achieving program objectives and KPIs, driving impactful outcomes for transitioning young adults.

As part of our internship program, you will receive hands-on experience, attend orientation, and have the opportunity to network with our staff. Our internships offer the potential for academic credit, depending on your institution. Also, High-performing interns may be considered for employment post-internship.
