Case Manager

The person in this role will coordinate the services for young adults in transition. This may include working with various agencies, providers, faith-based organizations, and individual partnerships. They will develop, implement, and manage the individualized plans for young adults ages 16-24. They will help to ensure young adults are provided with the tools, support, and resources necessary to become self-sufficient.

The Transition Specialist – Outreach (TSO) will be responsible for building relationships, awareness, and support to disconnected youth on the streets as well as through contact with other service providers, schools, and community centers. Outreach is done both in a vehicle and on foot in targeted areas throughout The Relatives’ service area. The TSO helps young people by reducing the risks to being victimized, improving their health, and connecting them with needed services and support.

This position is responsible for offering appropriate intervention services; facilitating individual, group, and family conferences; and providing appropriate referrals to residents and families as needed; responsible for maintaining the residence and clients in a clean and safe environment.
